We are seeking a Community Health Nurse to provide a variety of professional population-focused community and public health nursing services in health promotion, prevention and disease control programs; and to perform related duties as assigned. 


This position is within our Transition of Care and Access (TOCA) clinic. The Transition of Care and Access Clinic is designed to create an avenue for improved access, quality visits, efficiency, and support with follow up care and improved timely access to care for patients recently discharged from the hospital. The TOCA RN connects with the patients to obtain important information and provide triage to determine how urgently the patient should be seen. 


Description of TOCA RN duties: 

  • Retrieve Hospital Discharge Records 

  • Complete intake template   

  • Update and reconcile medications and allergies 

  • Update Health Histories 

  • Questionnaires – SDOH/PRAPARE 

  • Follow Up Care (PCP, referrals, DME/Meds, etc.) 

  • Completes LACE score – assessment tool that measures length of stay, acuity, co-morbidities, and number of emergency department visits in the last 6 months. 

        *This assessment identifies patients who are at risk for readmission or death within 30 days of discharge.


This activity supports the care team and provider to focus on the delivery of patient care, developing a supportive care plan around improved outcomes, and engaging in patient education. 


Brief Description of duties: 

  • Performs independent, comprehensive telephone and clinical triaging, and facilitates care coordination through a multidisciplinary approach. 

  • Provides patient education as an integral part of the services offered. 

  • Acts as relief nurse, triages and screens walk-in patients to assess the severity of the patient's condition and the need for provider intervention. 

  • Communicates with providers regarding determinations and situations, documents events appropriately. 

  • Other duties as assigned.

The Community Health Centers of Lane County provide affordable primary care for the whole family – infants, children, adolescents, adults, and seniors – as well as integrated behavioral health, alternative services such as acupuncture, and preventative dental services. At Community Health Centers, we focus on integrated care that values the whole person. Our services encompass not only treatment and prevention of illness, but also health education and behavioral health services. 


At Lane County Community Health Centers, we have adopted a model of primary care called Patient Centered Medical Home. Our services are organized around care teams who work collaboratively to support our patients. Our integrated care teams include primary care providers, nurses, medical assistants, patient care coordinators and behavioral health specialists. This model of primary care prioritizes the patient, who has increased access to other services beyond general medical care at our clinics. Our care teams also help patients reach their optimal health status by addressing social determinants of health barriers to care such as transportation, food insecurity, or housing.



  • While this position is posted at a primary location it will require the ability to float to our other clinics as needed to provide coverage, training, and support.

  • Please note - travel between all our clinic locations will be required during on-boarding period.

Job Requirements:

Community Health Nurse 1: Training & Experience:

  • Associates degree in Nursing from an accredited college or university is required.
    College-level coursework in the social sciences, and additional nursing training within a community or public health curriculum desirable. 
  • Nursing experience in public health or community health care services desirable. 

Community Health Nurse 2: Training & Experience:

  • Bachelor of Science in Nursing degree from an accredited college or university.

Or any equivalent combination of experience and training that provides the required knowledge and abilities is qualifying.


Special Requirements:
Please note the following will be required for credentialing:

  • State of Oregon Nursing License
  • Diploma 
  • NPI (downloaded copy acceptable) NPI should reflect Oregon address by time of hire 
  • Current BLS/CPR 
  • Valid Photo ID
