Job Requirements of Community Health Nurse 1 or 2:
Community Health Nurse 1: Training & Experience:
Associates degree in Nursing from an accredited college or university is required. College-level coursework in the social sciences, and additional nursing training within a community or public health curriculum desirable.
Nursing experience in public health or community health care services desirable.
Community Health Nurse 2: Training & Experience:
Bachelor of Science in Nursing degree from an accredited college or university.
Any equivalent combination of experience and training that provides the required knowledge and abilities is qualifying.
Special Requirements:
- State of Oregon Nursing License- (copy of original or web document)
- Diploma (copy of original is fine)
- NPI (downloaded copy acceptable) NPI should reflect Oregon address by time of hire
- Current BLS/CPR (copy of original is fine)

We are seeking a Community Health Nurse to provide a variety of professional population-focused community and public health nursing services in health promotion, prevention and disease control programs; and to perform related duties as assigned.
Brief Description of duties:
Performs independent, comprehensive telephone and clinical triaging, and facilitates care coordination through a multidisciplinary approach.
Provides patient education as an integral part of the services offered.
Acts as relief nurse, triages and screens walk-in patients to assess the severity of the patient's condition and the need for provider intervention.
Communicates with providers regarding determinations and situations, documents events appropriately.
Prepares and administers oral, subcutaneous, intra-muscular and intravenous medications.
Independently instructs and demonstrates to patients how to provide self-care.
Instructs patient in self-medication of prescribed pharmaceutical agents.

The Community Health Centers of Lane County provide affordable primary care for the whole family – infants, children, adolescents, adults, and seniors – as well as integrated behavioral health, alternative services such as acupuncture, and preventative dental services. At Community Health Centers, we focus on integrated care that values the whole person. Our services encompass not only treatment and prevention of illness, but also health education and behavioral health services.
At Lane County Community Health Centers we have adopted a model of primary care called Patient Centered Medical Home. Our services are organized around care teams who work collaboratively to support our patients. Our integrated care teams include primary care providers, nurses, medical assistants, patient care coordinators and behavioral health specialists. This model of primary care prioritizes the patient, who has increased access to other services beyond general medical care at our clinics. Our care teams also help patients reach their optimal health status by addressing social determinants of health barriers to care such as transportation, food insecurity, or housing.
