Community Health Nurse 2

$40.05 - $53.53/Hour



About the Position

The Lane County Public Health (LCPH) Communicable Disease section serves to prevent and control the spread of communicable diseases through evidence based public health practice. The Community Health Nurse will provide a variety of professional population-focused community and public health nursing services in health promotion, prevention and disease control programs; and to perform related duties as assigned. This position will provide direct program services related to immunizations, STI/HIV infection control, TB case management, and epidemiological work on reportable illness and communicable disease outbreaks. 


The immunization program includes community education, advocacy, and evidence based practice that aligns with a developed work plan for improving vaccine rates in Lane county. There are also expectations around vaccine administration at outreach events through community partnerships, as well as, in the clinic. 


The Sexual Health program includes assessment, testing, and treatment, along with education for management of STI/HIV infections, as well as providing consultations to community providers. These services are provided through the clinic and various outreach and tabling events in the community. 


TB case management is an intensive program offering consultation, contact investigation, infection control, and case management for patients with active TB. These services are often in a client's home during initiation of isolation and direct observation therapy. 


The Community Health Nurse offers clinical guidance and support to the disease outbreak team, community healthcare providers, stakeholders, and community members, occasionally taking the lead during outbreaks when necessary. This support is provided through phone consultations and a state database, in partnership with epidemiologists at both the county and state levels. Collaboration extends to Environmental Health, hospitals, long-term care facilities, daycares, schools, and laboratories, etc.   


About the Division

The Lane County Public Health (LCPH) Communicable Disease section serves to prevent and control the spread of communicable diseases through investigation, providing immunizations, education and treatment.


Schedule: Monday - Friday; 8:00am - 5:00pm

Job Requirements:

Community Health Nurse 2: Training & Experience:

  • Bachelor of Science in Nursing degree from an accredited college or university.

Any equivalent combination of experience and training that provides the required knowledge and abilities is qualifying.

Special Requirements:

  • Degree in Nursing from an accredited college or university 
  • Possession of a current State of Oregon license as a Registered Nurse at the time of appointment 
  • National Provider Identifier 
  • Possession of a valid CPR card 
  • Immunization records/TB test within 10 days of hire

  • This position is subject to a full criminal offender information record check. 

  • Offers of employment are contingent upon consenting to and successfully passing a drug screening test. 

  • Please note that as part of the screening process Lane County will verify license and certification status.

DRIVER’S LICENSE
Must be in possession of a valid driver's license at time of application, and a valid Oregon Driver's license by time of appointment.
