Office Assistant 2 - Part time
About the Position

If so, we have an exciting opportunity for you to join a talented and passionate team! We are seeking a detail-oriented and organized Office Assistant to support health-related reentry services in an adult correctional facility. The Office Assistant plays a vital role in helping incarcerated individuals transition back into the community by facilitating medical record transfers, coordinating referrals, and supporting continuity of care. A key focus of this role is ensuring that eligible individuals have active Medicaid coverage upon release and assisting uninsured individuals with Medicaid applications, when appropriate.
Key Responsibilities:
Coordinate the secure and timely transfer of medical records to external providers, Community Health Centers, and hospitals as part of pre-release planning.
Support reentry staff by processing and tracking health-related referrals.
Identify individuals who were enrolled in Medicaid prior to incarceration and initiate the process to reactivate their benefits prior to release.
Assist individuals who were uninsured prior to incarceration in completing Medicaid applications, including verifying eligibility and gathering necessary documentation.
Maintain accurate and confidential records related to Medicaid status, medical records, and referral processes.
Work collaboratively with health services staff, community-based organizations, state agencies, and other reentry personnel to coordinate services.
Respond to inquiries from internal staff and outside agencies regarding medical records and benefits coordination, within scope of role and in compliance with privacy regulations.
Track and report key metrics related to Medicaid reactivation and application assistance.
Assist with general administrative duties including scanning, filing, data entry, and managing secure communications.

The work is done collaboratively within a multidisciplinary team that includes a registered nurse case manager and peer support specialists as well as partnering with other social service providers in the community. This position will be embedded in the Lane County Adult Correctional Facility.

The Community Health Centers have been providing quality primary care and behavioral health services to the citizens of Lane County since 2004. Our clinics are Federally Qualified Centers whose mission is to serve the community by offering excellent care and eliminating barriers to healthcare. Our staff must show dedication to providing care with compassion and willingness to work with a diverse population.** Please note - travel between all of our clinic locations may be required during on-boarding and training period **
Schedule: Monday – Friday; 8:00am – 12:00pm
